YET ANOTHER VETERAN HELPED FOR CHANGE OF NAME AND DOB OF HIS WIFE

                VETERAN   SUB. JEBADOSS WITH HIS WIFE AT OUR TRUST OFFICE.

On coming to know the services of our Welfare Trust, , one more veteran called on our Trust Office for change of name and DOB of his wife:

NAME:  JEBADOSS             Rank: Subedar    Service No: JC 692003 H.

Record Office:   A M C  Lucknow.

Name of his wife as per  PPO/ Service Record  is  :JEBA RANI.  and  DOB is only year 1963., no date.

But as per School certificate, Aadhar and PAN card  , Name   SAKUNTHALA V.   and 

Date of birth is  30-09-1963.

Hence, we prepared the required documents  like,  

AFFIDAVIT,   SELF DECLARATION ,  and   arranged for  2 News paper Publication for change of Name and DOB.

Then prepared an application by the veteran, , attaching,  Affidavit, Self Declaration, News paper Publication, copies of   SSLC certificate,  Aadhar and PAN card .

We have guided the veteran to forward the application to his Record office, through  our Zilla Sainik board..

the veteran and his wife  were very happy with  the noble service and thanked us .

Our Trust is also happy in helping one more veteran.
