On coming to know the activities of our Trust, the below mentioned Veteran along with his wife visited our Trust office for changing the Name and DOB of his wife.
Name: R.VELMURUGAN RANK: HAVILDAR Serv.No: 15767741 H
Record Office: ARMY AIR DEFENCE ARTY RECORDS, GOPALPUR, ODISA.
PPO.NO: S// 37130/2014
Name of his wife is mentioned in the PPO as V.DHANAM JOYCE and DOB 30-05-1978.
Bur as per the documents like School Certificate, Aadhar card and PAN card ,
her Name is THANAM P AND DOB 31-05-1978.
Now, for change of name and DOB as per Army HQ instructions,
we prepared an AFFIDAVIT, Self Declaration , 2 News paper Publication and an application in this regard.
We advised the veteran to Forward the documents , Affidavit, Self Declaration by Spouse,
copies of School certificate, Aadhar and PAN card , PPO& Discharge book and News paper Publication to his Record Office through our ZSB, Tirunelveli Dist, TN.
The veteran and his wife were very happy with our service and guidance and thanked us for this valuable help.
Our Trust is also happy in helping one more veteran in this regard.
